In a move that will surely baffle the pharmaceutical industry, a drug that costs less than ten cents a day has been shown to significantly help heart failure patients. Three studies led by UMCG cardiologists Dirk Jan van Veldhuisen, Kevin Damman, and Peter van der Meer found that low-dose digoxin can reduce hospitalizations by 25% and potentially save lives. The findings, published in Nature Medicine and the Journal of the American Medical Association (JAMA), were also presented at the ESC Heart Failure Congress in Barcelona.
Heart failure, a condition where the heart can't pump blood effectively, affects over 500,000 people in the Netherlands alone, and that number is expected to rise. Standard treatment relies on a combination of four medications, aptly named the 'Fantastic Four.' Digoxin, the oldest and cheapest kid on the block, has been around for centuries, but its use has declined as newer, pricier drugs emerged. Only about 15% of heart failure patients currently receive it.
The UMCG studies provide strong evidence that digoxin deserves a comeback. One study followed 1,000 patients at 43 centers across the Netherlands, half of whom received low-dose digoxin for an average of three years. While the reduction in cardiovascular deaths (19%) didn't reach statistical significance on its own, a meta-analysis combining data from two earlier studies showed a clear, statistically significant benefit. Hospitalizations for heart failure dropped by an average of 25%.
But here's the twist: stopping digoxin might be a bad idea. A third study followed about 600 of the original participants and found that those who stopped taking digoxin experienced significantly more problems in the first six weeks. Among 288 patients, 14 were hospitalized or died. The researchers found this effect both impressive and surprising, though it doesn't directly prove digoxin's effectiveness.
So why did digoxin fall out of favor? Higher doses used to be prescribed, which made the heart muscle contract more strongly - but that proved less helpful for a weakened heart. Lower doses work by suppressing stress hormones like adrenaline, which is gentler on the heart. Previous research hinted at this, but randomized, prospective studies hadn't confirmed it until now.
The researchers hope these findings will influence guidelines and make digoxin available to more patients. With a price tag of less than ten cents a day, compared to several euros for newer medications, it's a bargain that could save healthcare systems a fortune. But funding research on cheap, old drugs is tough. The Hartstichting (Dutch Heart Foundation) stepped up with 3 million euros through its collaboration with ZonMw as part of the Good Use of Medicines program. Because apparently, saving money and lives isn't always a priority.
